If you were hired after January 2000 by the Chicago Transit Authority in the following classifications:

Customer Assistant, Rapid Transit Operator, Car Repairer, Motor Blower, Car Servicer, Rail Janitor, Trackman, or Material Handler,

Please print and fill out both the Application for Membership and the Check-Off Authorization form and mail to:
